Individual people need individual attention; that's where consultations come in!
WISH Health Counselors will spend one-on-one time with you to assess your individual needs though blood chemistry, diet and lifestyle analysis.
Initial consultations last for an hour and a half. If you're not sure that our services are right for you, you may schedule a ten minute phone consultation with Dr. Bevacqua, at no charge, by appointment only.

New Clients
For those who would like individualized programs tailored to assist them in attaining their healthcare goals, please follow these steps:
1. Contact our office to schedule an appointment. Wait times may vary from several weeks to several months.
2. Please download and complete the New Client packet.
3. Obtain any recent blood work from your physician. Your doctor can also fax copies of your blood work, pathology reports, etc. to our office directly.
If you do not have recent blood work please print out a copy of the blood work request sheet and take this to your primary care physician to request an order for the blood draw. Be aware that your physician may be unable to provide you with an order for all of the tests listed. Please allow 2-3 weeks for the lab to process your blood test results.
If you do not have insurance or your doctor will not write an order for the blood tests you are requesting, you may use our Blood Test Order Form if you wish to pay for the blood tests you would like to have run out of pocket at a discounted price. Plan on ordering blood tests at least one month prior to your appointment to allow for paperwork and lab processing.
4. Please return the completed forms and blood test results to our office at least one week prior to your appointment. Those who send paperwork in early become eligible for the cancellation list.
Consultations occur either over the phone or locally in the office. Distance is not a problem, out of town clients do not have to travel to Tucson. *Please call to schedule an appointment prior to having blood drawn, often there is a long waiting list to meet with Dr. Bevacqua. If you are unable to keep your appointment, please notify us at least 48 hours in advance, failing to do so may incur additional fees.

Payment Policies
Payment is expected at the time services are render and is required prior to shipping of any products. We accept Visa, Mastercard, Discover or American Express. Cash, check or money order. We do not accept insurance.
