New Clients

Start by booking a free 15-minute consultation with Dr. Sandy.  This allows you to talk with her one-on-one.

 

Should you and Dr. Sandy decide to work together, download and fill out the "New Client Packet" then return it to the office along with any requested test results that you have had within the past year. Please have the information submitted to the office 7 days prior to your initial consultation.

 

If you need testing outside of your primary care physician, please notify the office.

You may obtain recent blood work results from your healthcare provider. Your provider's office can also fax or email copies of your blood work and any other related reports to our office directly.

If you do not have recent blood work please print out a copy of the blood work request sheet and take this to your primary care healthcare provider to request an order for the blood draw. Please allow 2-3 weeks for the lab to process your blood test results.

Cancellation Policy For Initial Consultations

If you are unable to keep your appointment, please notify us at least 5 business days in advance, failing to do so may incur additional fees.

Please note, there is a place to write supplements down in the new client packet, but if you'd like to provide additional information regarding your supplements or use a chart to track your supplements please feel free to use this chart.

If you do not have insurance or your doctor will not write an order for the blood tests you are requesting, you may use our Blood Test Order Form. In this way, blood tests may be purchased through a third party vendor out-of-pocket at a highly discounted price.

Emergency consultations generally occur over a weekend and are subject to availability.
